What is Community Care?

Community care, also known as care in the community, is a policy which it aims to keep patients in their homes for as long as possible, instead of providing care in an establishment or care home.

Services vary depending on who is delivering the care and what your needs are, however, private carers will come to your home and carry out the support you need to ensure you live as comfortably as possible. Care may be funded by the local council or you may have to fund the care yourself, depending on your financial status and the results of a care needs assessment.


What is a Community Care Assessment?

According to the NHS, a Care Assessment helps to understand “how you’re managing everyday tasks like washing, dressing and cooking.” The assessment is designed so that any help you may need with such tasks can be identified and support planned.

A council employee such as a social worker or occupational therapist will assess with you which will last approximately an hour, either face to face or over the phone. If you or your loved ones think you need extra support at home then it’s important to ask for a care assessment to ensure you get the help you require. This could include having equipment installed in your home such as a walk-in shower, providing you with a walking frame or personal alarm, enabling you to access day centres and other community facilities or having a professional carer come to your home.
