What is a Respiratory Care Plan?
According to the NHS, “Respiratory disease affects 1 in 5 people and is the third biggest cause of death in England.” Therefore, there are many people living with respiratory disease who will need extra support and care during their lifetime. That provision should be planned in advance as it ensures that everything the person needs will be put in place before their condition deteriorates. Respiratory care plans contain all of the information needed to ensure that a person’s wellbeing is always at the forefront of their care planning, whether they are being supported in hospital or at home. Not everyone realises that complex, nurse-led care can be delivered in their own home just as lower-level support can be, and that’s why Helping Hands are dedicated to ensuring that everyone who needs us is able to access our exceptional visiting and live-in care services.
Respiratory Care Plan Example
Respiratory care plans will include information about the person’s daily routine, and physical requirements. This could be their medication, personal care and mobility needs, as well as nutrition and hydration preferences. It will also have information about the condition you are living with, the wider healthcare team who are supporting you and how to take any medication that they’ve been prescribed. It will also detail when the respiratory care plan should be reviewed and who will monitor it. Respiratory care plans can support people living with various respiratory conditions, such as:
Asthma
Chronic obstructive pulmonary disease (COPD)
Pneumonia
Pulmonary hypertension
Bronchiolitis
Lung cancer
Why Should I Have a Care Plan?
Cases of respiratory disease have risen in recent years; in fact, the NHS tells us that “Hospital admissions for lung disease have risen over the past seven years at three times the rate of all admissions generally.” Therefore, with cases rising, it’s so important that you have a breathing care plan if you are living with respiratory disease that will detail all of the support you require.
Having a care plan in place doesn’t mean that your condition is going to deteriorate or that your life expectancy will be significantly impacted. All it means is, should your condition get worse, there is sufficient provision in place to ensure that you have all the care and support you need.
What Does an Effective Care Plan for a Respiratory Customer Include?
An effective care plan will contain detailed information about treatments and therapies that you will have been offered for your condition. Our carers will be able to support you with many of these, managed by our own team of clinical nursing leads, including BiPAP (Bilevel Positive Air Pressure) care and CPAP (Continuous Positive Airway Pressure). It’s also very important to have information about both the person and their carer in the care plan as it’s important to know that the carer is experienced in delivering respiratory care support or has been given the appropriate skills. As well as their physical care needs the plan will usually include information about their emotional needs and requirements plus spiritual preferences, such as whether they go to church and whether they’d like a religious service when they pass away.
How Can I Create My Own Care Plan?
While you will be heavily involved in your own care planning, it’s important to talk to your wider healthcare team and produce it in conjunction with them. This ensures that it’ll be person-centred and that all of your needs and routines will be included. The Social Care Institute for Excellence (SCIE) explains that a care plan should be “developed with the person. The conversation is led by the person who knows best about their needs and preferences.” You should always be kept at the centre of your care-planning process and when you have support from Helping Hands, we’ll make sure that happens.
