Diabetes Care Plans
What is a diabetes care plan?
Diabetes is a condition that causes person's blood sugar level to become too high.
According to the NHS, Type 1 diabetes is a lifelong condition where the body’s immune system attacks and destroys the cells that produce insulin. Type 2 diabetes is when the body does not produce enough insulin, or the body’s cells do not reach insulin properly. Type 2 is far more common and over 90% of adults with diabetes in the UK have this type. A diabetes care plan will make sure to understand your diabetes and work around your condition. Diabetes UK outlines some symptoms of diabetes as:
Going to the toilet a lot
Feeling more tired than usual
Losing weight without trying to
Blurred eyesight
Increased hunger
Feeling very thirsty
Cuts and wounds taking longer to heal
Genital itching or thrush
Why should I have a care plan?
A diabetes care plan can help make positive changes to your life.
The NHS says that certain people are more at risk of developing diabetes. They include overweight and obese people, those who have a family history of the condition, are Asian, Black African or African Caribbean, have high blood pressure or have had gestational diabetes during pregnancy. The BHF advises that adapting your diet and reducing the amount of high-sugar, high-fat and high-salt foods is best. You should also exercise around two and a half hours per week and manage your blood pressure and cholesterol levels.
